Verified Partner Forms
The fillable version of the drop-off form is convenient to complete on the computer or you can print it and fill it out by hand. At this time CRA still requires penned signatures from CVITP clients.
The Newcomer Supplement Form is for when the client is a newcomer to Canada for any of the tax years requested to be filed. It contains additional information needed to prepare and file a newcomer’s first tax return.
The Document Request Form is for when a client’s taxes have already been filed, but they are in need of documents like an Option C (Proof of Income Statement) for benefit applications and housing, etc.

Each person having a return filed must sign both an Authorization signature page and TIS60. The drop-off form already has one of each attached. If needed, print off additional copies for the household you are supporting.
The fillable version of the TIS60 and Authorization signature pages can be completed on the computer or you can print it and fill it out by hand. Note that client signatures must still be penned as required by CRA.
Submitting Documents
You have three options for getting us the drop-off form, signed authorization forms, and other supporting documents (ID verification, copies of rent receipts, etc).
Once we have received your client’s drop-off form and documents, if we need to contact you or your client for more details, we will do so. Otherwise we will complete the taxes within 10 business days of receiving them and mail back the completed tax documents and any original documents (please send copies where possible as we are not responsible for anything lost in the mail) that were sent to us according to the mail directive. Please note that we do not keep a copy of the client documents, and all digital or physical copies are destroyed.

SUBJECT: Client Last Name, Client First Name - Drop-Off
MESSAGE: Your name, email, and phone number. Any additional details about the client if necessary.
ATTACHMENTS: Upload all necessary documents
Be sure to include:
Completed drop-off form (for all members of household where applicable) with client and drop off partner signatures where needed.
Signed TIS60 and Authorization form for all members of household needing taxes filed.
Copies of any slips, receipts for eligible medical expenses (client can get print out from pharmacy), charitable donations, rent, etc.
We should be able to get any T-slips issued by an employer or the government for a client through RepID.

Put your documents in an envelope, and clearly label the outside with your name and write the words “Partner Drop Off” on the outside. Our office is located at 294 Portage Avenue (in the Somerset building across from the Dollarama) on the 5th floor. Suite 500 is in front of you when you step off the elevators, and there is a mail slot and bin to the right of the door that you can put your clearly labelled envelope through.

The ‘dismissed’ status indicates that we have seen your request and have assigned it to someone to be completed. Dismissing it on our end removes it from our inbox to get out of the way of the other incoming items (it’s like moving it to a done folder).
There are a few other status indicators as well that you may see including opened, pending, completed, and unopened. Unopened means we have not yet read the message/request. Any of those other statuses means that the request is in process – we’re reviewing it, or waiting to send it to an available volunteer. As mentioned above, dismissed means it has been assigned and we are working on it. -
Ten business days is our service window to complete requests. We are often much faster than this!

Pretty much year round! With the exception of an approximately month long closure at the end of January due to CRA changing over their systems, and a few brief breaks around holidays, this program operates year round.
